- Your Name (This is your personal name)
- State: "I have been Elected (or Appointed) by my FGBMFI Chapter, to be the Web Servant for my Chapter."
- Your Email Address (This is where we will answer your questions)
- The Location of YOUR FGBMFI CHAPTER. (Examples: South Chapter Los Angeles, California, USA || Paris, France || Dumaguete City, Philippines || Springfield, ON, Canada)
